To report incidents or concerning behavior taking place at 麻豆原创, review the forms and descriptions below, select the most appropriate report, and complete the form.

Report any possible violations of 麻豆原创s policy prohibiting harassment against any student on the basis of sex or gender. Harassment of a student includes unwelcome sexual advances/conduct, sexual violence, and stalking.

Report acts of discrimination against a student on the basis of sex, gender, race, color, national origin, religion, or disability that adversely affects the student.

The College District prohibits retaliation against any person who in good faith makes a report or complaint, serves as a witness, or otherwise participates in an investigation, proceeding, or hearing under policies (FFDA, FFDB).

File a complaint/grievance regarding 麻豆原创 Faculty/Instruction or 麻豆原创 Department/Staff. This form must be submitted within 15 days of the date the student/complainant first knew or with diligence should have known, of the decision or action.

File a formal course grade complaint.

Appeal a result of a hearing on cases involving sexual misconduct, harassment, discrimination, and retaliation. The Code of Conduct affords both the complaining party and responding party the opportunity to appeal a decision within grounds.

Appeal a student complaint decision OR lack of timely response after a conference. Appeals will be heard in accordance with Policy FLD.

Student Support Forms

Apply for emergency assistance with our Chaps Helping Chaps Student Emergency Fund program while participating in 麻豆原创 programs. This fund is available to offer short term assistance that allows students to remain enrolled during times of need.

Apply for a laptop loan or hotspot loan through the SOS program. FIRST COME, FIRST SERVED! Unfortunately, there is a limited supply of both items. 

Apply for an order with The Chap Pantry while participating in 麻豆原创 programs. The Chap Pantry allows students to receive items in times of need. The inventory is limited and focuses on essentials like hygiene, basic needs, with a small selection of food items.

Apply for accommodations for a student with disabilities. 麻豆原创 is committed to providing equal and integrated access for their students. An application is required for EACH term you are enrolled, if you want to continue use. Students who have already submitted an application and documentation for accommodations and need to apply for the next term, please complete this Accommodations Renewal Form.

Counseling and mental health services are available to all enrolled 麻豆原创 students. Complete this application if you are a student requesting an appointment with our on campus LPC, or if you would like to refer a student to counseling services.

Use this form if you are or were at any point a part of the Foster Care System. To be eligible documentation must be provided and students must be currently enrolled and registered for courses at 麻豆原创. 

Faculty & Staff Forms

Notify pathways advisors when a student is exhibiting challenges that may impact their academic success. Early communication helps connect students with appropriate support services. Please fill out the form, and an advisor will be in touch!

Report acts of discrimination against a student or employee on the basis of sex, gender, race, color, national origin, religion, or disability that adversely affects the person.

The College District prohibits retaliation against any person who in good faith makes a report or complaint, serves as a witness, or otherwise participates in an investigation, proceeding, or hearing under policies (DIAA, DIAB) 

Report concerning behaviors and/or conduct matters to the 麻豆原创 behavior intervention team (CARE Team). Typical concerning behavior could be emotional or physical behaviors, unexplained absences, deteriorating appearances, and/or aggression. 

Report alleged violations of Student Conduct. Non-academic violations include alcohol and/or drug abuse, disorderly conduct, harassment, vandalism. 

Report academic student misconduct such as academic misconduct, plagiarism, cheating, or anything regarding student records to the Dean of Student Life.

Document an incident taking place in the 麻豆原创 housing to be used by residence hall managers, front desk clerks, and student residents.
